| Abstract Alternative | Abstract Alternative: Objective: To determine the prevalence, causes, medication possession ratio, and drug expenditure waste of irrational omeprazole prescribing in a community hospital, and to design and evaluate a multidisciplinary intervention to promote rational prescribing and deprescribing. Methods: An action research study using the PDCA cycle was conducted at Wat Phleng Hospital, Ratchaburi Province, Thailand, from July 2025 to April 2026. The study was divided into four phases: Phase 1 involved a retrospective analysis of outpatient medication data from 2,039 patients; Phase 2 utilized a two-round Delphi consensus process involving 16 healthcare professionals; Phase 3 monitored outcomes for three months post-intervention; and Phase 4 presented the operational outcomes to the Pharmacy and Therapeutics Committee (PTC).Results: Pre-intervention data from October 2023 to September 2024 showed a prevalence of irrational omeprazole prescribing of 92.16%. The main causes were prescribing without clinical indication (66.95%), inappropriate treatment duration (17.35%), and excessive dosage (10.47%). Regarding the Medication Possession Ratio (MPR) among patients with appropriate indications, values for 4- and 8-week treatment were below the recommended range (0.8–1.2), while the use of medication for gastrointestinal ulcer prophylaxis in patients receiving NSAIDs was above the recommended range. These issues resulted in a total financial loss of THB 85,875. Using the Delphi technique, the two interventions receiving the highest consensus were: 1) establishing the dosage, duration, and frequency of prescribing in accordance with treatment guidelines, and 2) implementing a Drug Use Evaluation (DUE) system to assess the appropriateness of prescriptions. Post-intervention, the rate of rational drug prescribing increased from 33.00% to 56.60%, and the rate of drug discontinuation among patients receiving continuous medication increased from 15.22% to 40.74%. The MPR for the 4-week treatment regimen returned to the recommended range (0.8–1.2) but remained below the recommended range for the 8- and 12-week treatment regimens among patients with non-responsive typical GERD. Similarly, the use of medication for NSAID-associated gastrointestinal ulcer prophylaxis remained above the recommended range. However, the monthly financial loss decreased by 51.27% (from THB 2,847.35 to THB 1,387.65), and the completion rate of DUE assessment forms increased to 100%, reflecting healthcare personnel's acceptance of and cooperation with the implemented measures. Conclusion: The multidisciplinary intervention, developed through the Delphi consensus method by integrating prescribing restrictions with medication appropriateness reviews, proved highly effective in promoting the rational use of omeprazole. Furthermore, it successfully reduced unnecessary drug expenditure in a community hospital setting. |
