| Abstract Alternative | Abstract Alternative: Background and Objectives: For patients diagnosed with heart failure with reduced ejection fraction (HFrEF), sodium-glucose cotransporter 2 inhibitors (SGLT2i) have been shown to lower the risk of cardiovascular (CV) mortality and heart failure hospitalizations (HFH). Nonetheless, limited clinical evidence exists regarding their efficacy when combined with varying doses of guideline-directed medical therapy (GDMT). This investigation sought to determine if integrating an SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA offers non-inferior protection against adverse cardiovascular events compared to utilizing a high-dose RASi and beta-blocker in patients receiving MRA. Materials and Methods: This study utilized a retrospective observational design to evaluate clinical data from 334 HFrEF patients who underwent treatment between March 31, 2018, and March 31, 2024. The study sample was divided into two distinct therapeutic cohorts: 110 patients who received an SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA, and 224 patients maintained on a high-dose RASi and beta-blocker in patients receiving MRA. Inter-group comparative analyses were executed utilizing a non-inferiority analytical framework. The primary composite outcome of interest focused on the combined incidence of CV death and HFH, alongside isolated assessments of its separate components. Furthermore, the protocol featured a secondary outcome centered on evaluating the changes in estimated glomerular filtration rate (eGFR) between both patient groups. Results: Following the application of inverse probability of treatment weighting (IPTW) and multivariable Cox regression, a significantly elevated risk for the primary composite outcome was observed in the SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA group (adjusted HR 4.10, 95% CI 2.07–8.13; p < 0.001), thereby failing to establish non-inferiority against a high-dose RASi and beta-blocker in patients receiving MRA. The risk of CV death showed a trend toward being higher in the SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA group, although this difference did not reach statistical significance (adjusted HR 6.40, 95% CI 0.89–46.19; p = 0.065). However, the SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA group was associated with a significantly higher risk of HFH (adjusted HR 4.47, 95% CI 2.20–9.06; p < 0.001). Regarding the secondary outcome, the rate of renal function decline did not significantly differ between the groups, with an adjusted beta of 0.44 mL/min/1.73 m² per year (95% CI -1.64 to 2.53, p = 0.677). Conclusions: In the management of HFrEF patients, utilizing an SGLT2i combined with a low-dose RASi or beta-blocker in patients receiving MRA did not reach non-inferiority benchmarks when compared against a high-dose RASi and beta-blocker in patients receiving MRA regarding the combined risk of CV death and HFH. On the other hand, no significant variations were detected between the two cohorts in terms of eGFR alterations over time. Ultimately, these data emphasize that maximizing the dosage of background guideline-directed medical therapies remains vital and should not be substituted by merely adding an SGLT2i to suboptimal regimens. |
